Plane with
6 on board missing in Bahamas, official says

Aviation officials in the Bahamas say a search is ongoing for a small
plane that was traveling to the capital of Nassau and never reached its
destination. The twin-engine Piper PA-23-250 Aztec F
(N62769) was en route from the island of Andros when air traffic
controllers lost radar contact. There were
six people on board. The Kingdom Airways LLC,
owner of the plane, is a local airline and the pilot was Bahamian. All
occupants are dead including businessman Carter
Campbell. There was fog and low visibility around New Providence
at the time air traffic control first registered the plane on radar but
it's not known why the plane apparently turned back toward Andros.
In February 2018 the aircraft was recovered from waters adjacent to the
previous search location by a salvage team with assistance from
specialist divers of the Royal Bahamas Defence Force. “All major
components and parts of the aircraft were recovered.” said Chief
Investigator of the Air Accident Investigation Department Delvin Major.

soldiers die after Turkish military training aircraft crashes in Isparta

Three crew members aboard
the CASA CN-235 military training plane
(98-148) died Tuesday after the
aircraft crashed in Turkey's southern Isparta province. Rescue teams
reached the wreck of the aircraft at a remote location in Yalvaç
district. There was thick fog in the area, which was likely the cause of
the incident.
Turkish Armed Forces (TSK) said in a statement that the aircraft took
off for a planned training flight from the 1st Jet Base Command in
central Eskişehir province at 11:03 a.m. with two pilots and a
technician. Radar connection was lost with the plane around 12:50 p.m.,
and rescue teams reached the plane's wreck north of Lake Eğirdir around
2:30 p.m., it added.
The families of the killed soldiers Major Ümit
Karamustafa and Captain Ali Şahin
were informed by officials.

В турецкой Испарте потерпел крушение учебный военный самолет, в
результате чего погибли три члена экипажа.
По данным Генштаба ВС Турции, военный самолет типа CN-235 CASА
(98-148) выполнял плановый полет. В 11.03 он поднялся в воздух с первого
аэродрома турецких ВВС в Эскишехире. Приблизительно в 12.50 связь с ним
прервалась, сразу после чего началась поисково- спасательная операция. В
14:30 обломки самолета и тела погибших военнослужащих - 2-х пилотов и
бортинженера - были обнаружены к северу от озера Эгирдир в провинции
Испарта.
Предпологают, что крушение самолета связано со сложными климатическими
условиями – снегом и густым туманом.

crashed under unknown circumstances.
Paul Cobb was no stranger to a helicopter,
teaching others as an instructor and flying them while working for the
Pasadena Police Department. The 67-year-old, who was
co-pilot on
a helicopter headed to his friend's New Mexico ranch, didn't finish his
trip late Wednesday as he was among two men from the Houston area found
dead.
The group was traveling to the New Mexico ranch of
Cobb's longtime friend Charles
Burnett's, 61, a Houston businessman.
Jamie Coleman Dodd,
57, of Colorado, who was the pilot
of the helicopter, was also among
the dead.
Also among the dead were Zimbabwe opposition
leader Roy Bennett,
60, and his wife, Eileen Bennett,
55. Cobb's daughter, Andra Cobb,
39, was injured and the sole
survivor of the crash.
The aircraft was apparently traveling to Folsom,
New Mexico, less than 40 miles away. The injured victim of the crash
called 911 after the helicopter went down Wednesday evening near the
Colorado state line and authorities launched a search. The fiery
wreckage was found in a remote area near Raton in northern New Mexico.
Authorities on Thursday were investigating the cause of the crash.
Paul Cobb, a licensed pilot was named Pasadena
police chief in 2001. The Conroe resident met Burnett more than two
decades ago as an instructor teaching others to fly helicopters. The two
men stayed friends and often flew together. Paul Cobb briefly served in
the Army in Vietnam in 1970 and wanted to become a helicopter pilot, but
was shot and had to return home after just 13 days.
He went on to join the Pasadena police force,
where he often flew the department's helicopter.
Charles Burnett III was the wealthy British
eccentric best known for holding the world speed record for a
steam-powered car.
Some of Mr Burnett’s friends are questioning
whether last Wednesday’s fatal crash in New Mexico was accidental.
Suspicions have been raised because Roy Bennett, a leading opponent of
Robert Mugabe in Zimbabwe, was also killed. The 60-year-old Zimbabwean
was treasurer-general of Morgan Tsvangirai’s Movement for Democratic
Change. He had previously received death threats.
People are highly suspicious about the
circumstances of the crash,’ said
a friend of Mr Burnett. ‘The chopper went down on a clear night with two
highly qualified pilots on board. There has been talk in private about
Charles bankrolling Roy’s political ambitions. Was he planning to help
Roy launch an opposition campaign in Zimbabwe?’

military plane crashed in training exercise, at least 12 dead

An unidentified aircraft, operated by the Chinese
Air Force, PLAAF, was destroyed when it impacted terrain near
Zhengchang, Guizhou Province, China.
Little information was released by Chinese defense
officials. While the aircraft type was not disclosed, video footage from
the wreckage shows what seems to be part of a serial on a tail fin. This
would point to Shaanxi Y-8GX-3 (30513). The Y-8GX-3 is
an airborne command post and ECM version of the Y-8, which is a Chinese
development of the Antonov An-12. Chinese sources suggest that there
were 5 crew members and 7 military
personnel on the flight. There were no survivors.

Two people were killed when a transport helicopter crash-landed in
Siberia, a day after a passenger jet crashed in Moscow, killing all 71
people on board.
On 12.02.2018 at a distance of 100 km from Strezhevoy Airport (Tomsk
Region, RF), the accident occurred with Mi-8Т RA-22330 helicopter owned
by Yeltsovka Airline Company, CJSC. According to the available
information 3 crew members and 3 passengers were on board.
Two crew members were killed, one crew
member and three passengers were injured. The rotorcraft was
burned down.

Финальный отчет
Днем в понедельник, 12 февраля, из поселка Прохоркино в райцентр
Александровское Томской области, должен был состояться рейс санавиации.
Для северного населенного пункта – отвозить пациентов до районной
больницы дело привычное. На другой технике зимой к поселку не
подобраться, так что других вариантов, кроме вертолета, нет.
Экипажу предстояло доставить 45-летнюю пациентку Галину Миронову в
больницу. У женщины было подозрение на психическое расстройство.
На борту, кроме самой пациентки, ради которой организовали рейс,
находились трое членов экипажа, полицейский и фельдшер. Вылет состоялся,
но до пункта назначения вертолет не долетел.
В МЧС поступила информация о том, что в 14:46 по местному времени
сработал аварийный буй вертолета Ми-8, - пояснили в ГУ МЧС России по
Томской области. Связи с экипажем не было. На поиск воздушного судна
вылетели вертолёты Ми-8 из соседнего Каргасокского района, и еще один из
Стрежевого. В 17.30 рухнувший вертолет обнаружили в 42 километрах
юго-восточнее села Александровское Александровского района.
Двое человек – это члены экипажа –
погибли. Еще один член экипажа и трое пассажиров
доставлены в больницу города Стрежевой.
В реанимации были приняты двое пострадавших: член экипажа Григорий
Владимирович Шепелев 1960 года рождения, у него диагностирована
политравма (он из Стрежевого).
С таким же диагнозом находится и сама пациентка, которую
госпитализировали из Прохоркино.
В хирургическом отделении находятся двое других пострадавших: фельдшер
Дмитрий Сергеевич Сыренко 1985 года (он родом из Стрежевого, врачи
диагностировали у него закрытую черепно-мозговую травму, ушибы мягких
тканей, переохлаждение) и полицейский Андрей Николаевич Чуприна 1977
года рождения (он родом из Стрежевого, врачи диагностировали у него
закрытую черепно-мозговую травму, ушибы шеи, грудной клетки).
Вертолет Ми-8Т (RA-22330), принадлежит авиакомпании
"Ельцовка", экипаж 3 человека.
В компании изымается вся документация относительно этого вертолета.
Возбуждено уголовное дело по признакам преступления, предусмотренного ч.
3 ст. 263 УК РФ (нарушение правил безопасности движения воздушного
транспорта, повлекшее по неосторожности смерть двух лиц). Из
Новосибирска на место ЧП выехала следственная группа.
Во время расследования авиакатастрофы было выяснено, что в течение
периода времени 07:15:00 – 07:39:39 полет проходил в штатном режиме, без
отклонений и замечаний со стороны экипажа. В 07:30 КВС передал
управление второму пилоту. В 07:39:39 вертолет начал самопроизвольно
крениться вправо и за 3 с достиг крена 13.3°. На вопрос КВС о причинах
создания крена, второй пилот ответил, что управляющих действий не
производил. КВС взял управление вертолетом на себя. В 07:39:45 вертолет
опять стал крениться вправо и за 3.5 с достиг крена 31° с одновременным
увеличением угла тангажа до 9° на кабрирование. Из объяснений КВС, самопроизвольное кренение
вертолета вправо было определено им как неисправность в системе
поперечного управления.
При осмотре вертолета на месте АП было выявлено отсутствие болта
КАУ30Б-022 на штатном месте на КАУ-30Б системы поперечного управления.
Указанный болт является передающим звеном от ручки управления на
управляющий золотник КАУ-30Б поперечного управления, и его отсутствие
означает, что в системе поперечного управления произошло рассоединение,
и вертолет стал неуправляем по крену.
Следует отметить, что движение болта КАУ30Б-022 при сходе со штатного
места первоначально привело к возникновению небольших самопроизвольных
кренов (которые отмечены на расшифровке параметров полета, которые были компенсированы экипажем, так как произошло
рассогласование системы поперечного управления, а рассоединения еще не
произошло.
После рассоединения поперечного управления осталось постоянное
управляющее воздействие, кренящее вертолет вправо.
По результатам дешифрирования параметрического самописца, вертолет с
левым креном около 30°, с углом тангажа на пикирование 16° и
поступательной скоростью около 210 км/ч, ломая деревья, столкнулся с
землей.
После удара ВС подбросило вверх. Далее вертолет правым бортом столкнулся
с березой толщиной около 25 см, развернулся влево и
опрокинулся на правый борт. Наиболее вероятно, что при соударении
вертолета с землей и деревьями произошло разрушение топливных баков с
последующим возгоранием топлива.
Анализ дешифрирования средств объективного контроля, показаний КВС и
технического состояния ВС свидетельствует о том, что причина АП
техническая и связана с рассоединением системы поперечного управления
вертолетом из-за отсутствия болта КАУ30Б-022 на штатном месте на
КАУ-30Б.
Так как на момент утверждения данного отчета болт КАУ30Б-022, корончатая
гайка, шайба под гайку, две подпруженные шайбы, шплинт и контровка с
заводской пломбой обнаружены не были, однозначно установить
причину выхода болта со штатного места не представляется возможным.
Вертолет
Ми-8Т се разби в Сибир

Транспортен вертолет Ми-8Т (RA-22330) на новосибирската
компания „Елцовка” се разби в брезова гора на 100 км от
град Стрежевой и на 42 км от Александровское, Томска област, Русия.
Полета бил по маршрут с. Прохоркино – районен център Александровское за
превозване на пациентка с прояви на психическо разстройство. Освен нея
на борда били още 3-членният екипаж, фелдшер и полицай. Към 14:39 м. вр.
вторият пилот отбелязъл невъзможност да стабилизира крена на вертолета
при полета. Управлението поел командирът на екипажа и установявайки, че
има проблем с напречното управление, бързо взел решение за принудително
кацане, но се разбили. Вертолетът бил обхванат от пламъци.
Загинали вторият пилот и бордният механик.
Командирът на въздухоплавателното
средство и пътниците били открити своевременно от спасителните екипи и
откарани за лечение в болницата в Стрежевой. С най-тежки травми
е жената.
При разследването се оказало, че липсва болт КАУ30Б-022 от системата за
напречното управление КАУ-30Б, което прави невъзможно управлението на
вертолета по крен. Комисията заключава, че не може еднозначно да се
определи причината за разрушаването (изпадането) на болта. Същият не е
открит. Няма посочени виновни за катастрофата.

Russian
transport plane crashes at Syria base, killing all 39 on board

A Russian military transport airplane has crashed during landing at
Khmeimim Airbase in Syria around 15:00 Moscow time.
All on board 6
crew members as well as 33 passengers, all of them
military personnel, were killed in the crash.
The airplane hit the ground some 500 meters from the runway. The
incident could have been caused by a technical malfunction.
It did not come under fire before the incident, the Russian military
said, but the rebels belonged to Jaish al-Islam group took the
responsibility for the crash.
The Antonov An-26, also known as Curl under its NATO reporting name, is
a twin-engine turboprop aircraft designed as a multipurpose tactical
transport plane. It was developed in the Soviet Union in the 1960s. Some
450 such aircraft are still in service, with most of them used by the
Russian military.

Сирии разбился российский транспортный самолет Ан-26

Катастрофа Ан-26 в Латакии — крупная авиационная катастрофа,
произошедшая 6 марта 2018 года. Самолет Ан-26 (RF-92955 – 52
красный) разбился в 500 метрах от ВПП базы Хмеймим, в
результате все находившиеся на борту
39 человек погибли.
Самолет Ан-26, принадлежал к 6-й армии ВВС и ПВО (Санкт-Петербург).
Самолет использовался для внутренних сирийских перевозок, во всяком
случае, в Хмеймим он вылетел с Кувейриса, дважды садился на
промежуточных аэродромах (в том числе и Думейре), и только затем
направился в Хмеймим.
Самолет разбился при заходе на посадку в пункте назначения примерно в
15:00 по московскому времени 6 марта 2018 года, не долетев 500 метров до
взлетно-посадочной полосы. По данным Министерства обороны Российской
Федерации, на борту упавшего самолета были 33 пассажира (все
военнослужащие российской армии, из них 27 офицеров и сержантов, один в
звании генерал-майора) и шесть членов экипажа, все они погибли,
предварительной причиной была названа техническая неисправность. Один из
очевидцев в подробностях описал момент трагедии: "Самолет при заходе на
посадку резко увело в сторону, где-то через секунд семь его развернуло
обратно, затем он резко начал терять высоту, при ударе о землю судно
перевернулось и оторвалось крыло, оно влетело в теплицы местных жителей,
сам самолет понесло по земле дальше, пожар вспыхнул мгновенно".
7 марта исламистская группировка "Джейш аль-Ислам" взяла ответственность
на себя за сбитый самолет, но подтверждений этому не найдено. Сослуживцы
КВС рассказали, что наверно их сбили пулеметами из жилого микрорайона.
По другим сведениям, самолет был изготовлен в 1980 году, и причиной
катастрофы могла стать его изношенность. У участников расследования
крушения Ан-26 в Сирии появилась версия, подтверждающая технические
причины авиакатастрофы. Сообщается, что транспортный самолет мог не
дотянуть несколько сотен метров до аэродрома из-за повреждения
закрылков. Последние данные снимают с пилотов ответственность за
произошедшую трагедию.
Среди погибших — генерал-майор Владимир Еремеев, 1 полковник, 2 капитана
1-го ранга, 6 майоров, 29 младших офицеров и сержантов.
Из них, шесть членов экипажа:

Штурман — старший лейтенант
Константин Алтунин.

Руски военно-транспортен самолет
Ан-26 (RF-92955 – 52 червен) извършващ полет от Кувейрис за
Хмеймим с две промеждутъчни кацания се е разбил при кацане на 500 м от
началото на ПИК в авиобаза Хмеймим.
Няма оцелели от
намиращите се на борда 39 души,
офицери и сержанти, сред които и генерал-майор
Владимир Еремеев.
Според очевидци на произшествието самолетът рязко се отклонил встрани
при захода си за кацане. След 7 секунди отклонението му било
компенсирано, но започнал бързо да губи височина, ударил се в земята и
преобърнал, при което крило се отделило от фюзелажа и влетяло в
оранжерия на местни жители. Самолетът се плъзнал по-нататък и запалил.
По самолета не е забелязано да е воден вражески огън, но на 07 март
ислямистката групировка Джейш ал-Ислам заявява, че нейни бойци са
обстрелвали самолета с тежки картечници, което довело до катастрофата.
Колеги на командира на екипажа са споделили дискретно, че прелитайки над
близкия жилищен квартал самолетът е бил обстрелван със стрелково оръжие.
Самолетът е произведен през 1980 г и официалната версия за трагедията е
откъсване на задкрилка поради остарялост на метала,
т.е. техническа неизправност, което снема отговорността от летците за
катастрофата.

Six people were killed and one
was injured in a Russian Mi-8 helicopter crash
in Chechnya, after the rotor blades touched the ground causing the
aircraft to fall. There were 9 people on board. Two of them jumped just
before the crash. The aircraft belongs to the border troops. Six medical
teams from several Chechen regions are working in the site of the
accident. Kadyrov said "complicated weather conditions" may have caused
the crash on March 7 in the mountains of southern Chechnya.

В среду при заходе на посадочную площадку, находящуюся в горной
местности, вертолет войсковой части Ми-8 столкнулся с землей. По факту
крушения вертолета возбуждено уголовное дело по ст. 351 УК РФ (нарушение
правил полетов, повлекшее тяжкие последствия).
Вертолет пограничного управления, который базируется в Тусхарое
Итум-калинского района, полетел, чтобы отвезти пограничников на заставу.
"Вертолет снизился, чтобы высадить пограничников. Когда он завис в двух
метрах от земли, произошел резкий порыв ветра. Вертолет заволокло, двое
пограничников успели спрыгнуть. 7 человек оставались в вертолете, когда
он уже с обрыва высотой в 150 метров упал в пропасть", - написал Кадыров
в Telegram.
Из пропасти достали наверх пять тел
и двоих тяжело раненых бойцов,
один из которых
скончался в республиканском госпитале. Второй находится
в стабильно тяжелом состоянии под бдительным контролем врачей. "Я желаю
скорейшего выздоровления раненому бойцу, и приношу искренние
соболезнования семьям погибших", - добавил глава Чечни.

Вертолет Ми-8 на Граничната служба на ФСБ се е разбил
при опит за кацане в планински район. Предполага се, че сложните
метеорологични условия в района – сняг, дъжд и ограничена видимост – са
от факторите довели до катасгрофата. На борда е имало 9 души. При
кацането порив на вятъра поднесъл вертолета, той закачил земята с
перките на основния винт, обърнал се и разбил в пропастта. Двама от

A Turkish private jet Bombardier CL604 (TC-TRB),
belonged to Basaran Holding Company, flying a group of young women from
the United Arab Emirates to Istanbul crashed Sunday in heavy rain in a
mountainous region of Iran, killing
all 11 people on board. Days earlier, the doomed
aircraft carried a bachelorette party bound for Dubai. The plane hit a
mountain near Shahr-e Kord and burst into flames. Shahr-e Kord is some
370 kilometres (230 miles) south of the capital, Tehran.
Local villagers had reached the site in the Zagros Mountains and found
only badly burned bodies and no survivors. DNA tests would be needed to
identify the dead. Villagers said they saw flames coming from the
plane’s engine before the crash.
The plane took off from Sharjah International Airport late Sunday
afternoon and climbed to a cruising altitude of just over 35,000 feet. A
little over an hour later, it rapidly gained altitude and then dropped
drastically within minutes, according to FlightRadar24, a
flight-tracking website.
Basaran Investment Holding is active in the food,
finance, energy, construction, tourism and travel industries, according
to the company’s website.
Mina Basaran, the
28-year-old daughter of Basaran’s chairman who is part of the company’s
board of managers and is in line to run the business, recently posted
photographs on Instagram of what appeared to be her bachelorette party
in Dubai. Iranian emergency management officials said all the passengers
were young women.

helicopter (6WHTA) of the
Senegalese army with 20 people on board crashed Wednesday night in
southern Senegal near Missrah in the Mangroves area (Fatick region).
The toll is heavy: 8
dead, 12 people injured. The 12 wounded
have been sent to Kaolack Hospital for first aid. They were subsequently
taken to the Dakar Main Hospital. Three crew members perished in the
helicopter crash. They are Colonel
El Hadj Mamadou Toure, pilot of the aircraft, co-pilot Captain
Emmanuel Sarr, and the flight mechanic, Sergeant
Chief Thiandélla Fall.
A commission of
inquiry was set up to determine the circumstances of the accident.

service members were killed Thursday in a helicopter
crash in western Iraq. The HH-60 Pave Hawk helicopter
struck a power line and crashed along the Syrian border. Army Brig. Gen.
Jonathan P. Braga, the director of operations for the combined task
force leading the fight against ISIS in the region, said all personnel
aboard the helicopter were killed. The military said the crash, which is
under investigation, "does not appear to be a result of enemy activity."
The Defense Department released the names of the seven airmen on
Saturday. They are Captain Mark K. Weber,
29, of Colorado Springs, Colorado; Captain Andreas
B. O'Keeffe, 37, of Center Moriches, New York;
Captain Christopher T. Zanetis, 37, of Long
Island City, New York; Master Sergeant Christopher
J. Raguso, 39, of Commack, New York; Staff
Sergeant Dashan J. Briggs, 30, of Port Jefferson Station, New
York; Master Sergeant William R. Posch, 36,
of Indialantic, Florida; and Staff Sergeant Carl
Enis, 31, of Tallahassee, Florida. Two members of the New York
Fire Department were among those killed in the crash, later identified
as Raguso and Zanetis.
The HH-60 is a modified version of the Black Hawk helicopter. It is
often flown by the Air Force for medical evacuation missions and is used
by special operations para-rescue specialists.

Military Plane Crashes in Serbia: One Pilot Dead, Another Injured

The Serbian military aircraft Super Galeb (SOKO G-4
Super Galeb) crashed to the northeast of Belgrade near the town of
Kovacice. The two pilots ejected before the crash, but
one pilot of
the Technical Testing Center
died after ejecting, the co-pilot
sustained injuries. He is conscious and was transported to the
Military Medical Academy in Belgrade. Defense Minister Aleksandar Vulin
and Army Chief of Staff Gen. Ljubisa Dikovic visited the co-pilot in
hospital. An investigation has been opened about the incident.

On April 11, 2018, about 0818 Alaska daylight time, a single-engine,
turbine-powered Cessna 208B Grand Caravan airplane,
N814GV, impacted snow-covered terrain about two
miles north of the Atqasuk Airport, Atqasuk, Alaska. The
airline transport pilot, the only occupant onboard, sustained
minor injuries, and the airplane sustained substantial damage.
The airplane was being operated by Hageland Aviation Services, Inc., dba
Ravn Connect, Anchorage, Alaska, as a visual flight rules (VFR)
cross-country cargo/mail flight under 14 Code of Federal Regulations
Part 135. Visual meteorological conditions (VMC) existed at the flight's
point of departure, and company flight-following procedures were in
effect. The flight originated about 0758, from the Utqiagvik (formerly
Barrow) Airport, Utqiagvik Alaska, and it was destined for Atqasuk,
which is located about 58 miles south.
The area between Utqiagvik and Atqasuk consists of remote, flat,
featureless, tundra-covered terrain, which is snow-covered in April.
During a telephone conversation with the National Transportation Safety
Board (NTSB) investigator-in-charge (IIC) on April 12, the director of
safety for the company related that the purpose of the flight was to
transport a load of U.S. Post Office mail to Atqasuk. During a brief
telephone conversation with the NTSB IIC on April 13, the accident pilot
said that he departed from Utqiagvik just before 0800 with approximately
1,500 pounds of mail on board that was destined for Atqasuk. He noted
that weather conditions at the time of departure consisted of clear
skies, 9 miles visibility, and a light wind. After takeoff, the airplane
climbed to 2,500 feet msl, and proceeded southbound towards Atqasuk. The
pilot said that as the airplane neared Atqasuk, with the autopilot
engaged, the airplane descended to about 1,500 feet msl, but then he
noticed an area of low fog around Atqasuk. He said that when the
airplane was about two miles from the airport, he heard the audible
autopilot disengagement annunciator tone sound, which was immediately
followed by the pilot's control column pitching forward. The pilot said
that he was unable to pull the control column back, and the airplane
subsequently descended into instrument meteorological conditions. He
said that the airplane continued to descend into the fog, then it struck
the snow- covered tundra, and nosed over. The airplane sustained
substantial damage to the wings, fuselage, and empennage.
The airplane was equipped with a digital, 406 MHz ELT that instantly
transmits a distress signal to search and rescue satellites, thereby
alerting rescue personnel within minutes of the location of the crash.
However, no emergency transmitter locator (ELT) signal was received by
search personnel, but the pilot was able to use his cell phone to alert
company personnel of the accident.
During a telephone conversation with the NTSB IIC on April 13, a North
Slope Borough Search and Rescue helicopter pilot said that while en
route from Utqiagvik to the accident site he encountered ice fog,
reduced visibility, and flat light conditions that made it difficult to
discern topographical features on the snow-covered tundra. He noted that
as the flight continued, both pilots noticed ice beginning to accumulate
on the helicopter's windscreen, so the decision was made to abort the
search and rescue flight, and the helicopter returned to Utqiagvik. A
search team from Atqasuk eventually reached the accident site on snow
machines and transported the pilot to Atqasuk.
The closest recorded weather reporting facility was the Utqiagvik
Airport, 58 miles north of the accident site. The 0753 observation
reported, in part: Wind, 030° at 9 knots; visibility, 9 statute miles;
clouds and sky condition, clear; temperature, minus 7° F; dew point,
minus 3° F; altimeter, 29.66 inches of Mercury.
A detailed examination of the airplane wreckage is pending.

Killed In Helicopter Crash In Russia's Far East

Six people have died in a
helicopter crash in the Russian Far East city of Khabarovsk. The
Mi-8ПС-9 helicopter (RA-24640) operated by local airline Vostok
crashed on April 11. All six people aboard were killed, it said. There
were no reports of casualties on the ground.
The Investigative
Committee said the flight had been training and because of the poor
weather and bad visibility the crew hit a broadcast radio tower and
crashed. The helicopter caught fire and burnt.
Regional authorities
declared April 12 a day of mourning in Khabarovsk Krai, a region on the
Chinese border some 6,200 kilometers east of Moscow.

происшествие с вертолетом Ми-8ПС-9 (RA-24640)
АО "Авиакомпания «Восток".
По имеющейся информации, выполнялся
тренировочный полет, находившиеся на борту
шесть членов экипажа погибли, воздушное судно
разрушилось и сгорело.
В соответствии с российским воздушным законодательством
Межгосударственный авиационный комитет (МАК) сформировал комиссию по
расследованию данного авиационного происшествия.
Первый взлет ВС с целью тренировки первого КВС был выполнен в 01:01
11.04.2018. Полеты выполнялись в районе посадочной площадки по ПВП, по
кругу, на высотах 100 - 150 м по давлению ВПП.
После выполнения полетного задания первый тренируемый КВС покинул
вертолет, а место командира ВС для продолжения тренировки занял второй
КВС, после чего, в 01:15, был выполнен взлет по кругу с целью отработки
посадки с одним задросселированным двигателем. При выполнении полета по
кругу экипаж попал в ухудшившиеся метеоусловия, не сумел визуально
определить место начала третьего разворота, начал его выполнение с
запозданием, пролетев на 1600 м дальше расчетной точки начала разворота.
В процессе разворота, на высоте 140 м относительно уровня земли (150 м
относительно уровня ВПП2) и скорости 140 км/ч, в условиях ухудшенной
видимости произошло столкновение ВС с вышкой широковещательной
радиостанции (ШВРС), с последующим столкновением с землей. Вертолет
разрушился и сгорел, экипаж и тренируемые КВС погибли.
Причиной катастрофы явилось столкновение вертолета Ми-8П RA-24640 с
вышкой ШВРС на высоте 150 м при выполнении захода на посадку в условиях
ухудшенной видимости из-за выпадения ливневых осадков в виде дождя и
снега, что привело к разрушению конструкции ВС (воздушное судно) и его
падению.
Авиационное происшествие явилось следствием непринятия экипажем
своевременного решения о переходе на полет по ППП (правила полетов по
приборам) при ухудшении метеоусловий и наборе безопасной высоты полета с
целью исключения столкновения с искусственными препятствиями в районе
посадочной площадки.
Столкновению ВС с вышкой ШВРС способствовало неиспользование экипажем
возможностей СРППЗ (система раннего предупреждения приближения к земле)
при выполнении полета по ППП.
Ответственность авиакомпании "Восток" застрахована в компании "Согаз", а
пилотов - в "Сургутнефтегазе".
Власти Хабаровского края опубликовали список погибших. В нем значатся
проверяющий Алексей Штанько, три командира
экипажа - Дмитрий Макаров, Антон Ермаков и
Виктор Посохов, бортмеханик
Юрий Борода и второй пилот

Шест души загинаха
при катастрофа на вертолет в гр. Хабаровск, Русия, 6 200
км източно от Москва. Ми-8ПС-9 (RA-24640) принадлежащ
на авиокомпанията „Восток” катастрофира на 11 април
2018 година при изпълнение на тренировъчен полет за излитане, полет и
кацане с един дроселиран двигател.
На борда е имало първоначално 7 души, но след
изпълнение на упражнението първият обучаем е напуснал машината. На борда
останали инструкторът - командир на екипажа, вторият пилот, бордовият
механик и тримата обучаеми, които са също командири на екипажи от
авиокомпанията. Метеорологичните условия са били изключително динамични
с бързо изнасяща се към площадката за кацане ниска облачност, с усилващ
се валеж на дъжд и сняг бързо понижаващи видимостта и вятър с пулсации
до 14 м/сек. Инструкторът поисква разрешение за прекратяване на
упражнението предвид обстановката от зам. командира на ае, но той се
колебае да вземе решение и тръгва да съгласува предложението с
диспечера. Тъй като обстановката все още позволявала излитане и полет по
отношение на установените минимуми за района, инструкторът решава да
предприеме ново излитане с втория обучаем, но за минути видимостта рязко
се снижава до 1500 м и те пропускат да видят пътя ориентиращ ги
обикновено в СМУ (сложни метеорологични условия) за заход към площадката
за кацане. Започват захода със закъснение по кръга от 1600 м и попадат
на жарето (вишката) с антената за радиоизлъчване. Обозначителните лампи
на жарето са светели, казват очевидци. Видели са го в последния момент и
с остър крен са се опитали да го избегнат, но безуспешно. Вертолетът се
разбива и изгаря. Системата за ранно предупреждение за приближение със
земята била включена, но с изключена функция за звуково предупреждение,
вероятно поради това, че полетът е изпълняван на малка височина.
Полетът до последно се е провеждал по правилата за
визуален полет, а не за такъв по прибори. Миг нерешителност, миг
подценяване на динамиката в обстановката е коствало живота на 5-ма от
най-подготвените летци и борден механик на авиокомпанията.

On April 17, 2018, at 11:03 eastern daylight time, Southwest
Airlines flight 1380, a Boeing 737-700, N772SW, experienced a
failure of the left CFM International CFM-56-7B engine and loss of
engine inlet and cowling during climb about flight level 320. Fragments
from the engine inlet and cowling struck the wing and fuselage,
resulting in a rapid depressurization after the loss of one passenger
window. The No.13 fan blade had separated at the root. The flight crew
conducted an emergency descent and diverted into Philadelphia
International Airport (KPHL), Philadelphia, Pennsylvania. Of the 144
passengers and five crewmembers onboard,
one passenger (Jennifer Riordan, a mother
of two) received fatal injuries (row 14 nr the window)
and eight passengers received minor injuries. The
airplane sustained substantial damage. The regularly scheduled domestic
passenger flight was operating from LaGuardia Airport (KLGA), Queens,
New York, to Dallas Love Field (KDAL), Dallas, Texas.
Three flight attendants were assigned to the flight, and an additional
SWA employee was in a jumpseat in the cabin. During interviews, the
flight attendants and the employee reported that they heard a loud sound
and experienced vibration. The oxygen masks automatically deployed in
the cabin. The flight attendants retrieved portable oxygen bottles and
began moving through the cabin to calm passengers and assist them with
their masks. As they moved toward the mid-cabin, they found the
passenger in row 14 partially out of the window and attempted to pull
her into the cabin. Two male passengers helped and were able to bring
the passenger in.
During interviews, the flight crew stated the climbout from LaGuardia
was normal with no indications of any problems; the first officer was
the pilot flying and the captain was the pilot monitoring. They reported
experiencing a sudden change in cabin pressure, aircraft yaw, cockpit
alarms, and a “gray puff of smoke.” They donned their oxygen masks, and
the first officer began a descent. Flight data recorder data showed that
the left engine parameters all dropped simultaneously, vibration
increased, and, within 5 seconds, the cabin altitude alert activated.
The FDR also indicated that the airplane rolled left to about 40 degrees
before the flight crew was able counter the roll with control inputs.
The flight crew reported that the airplane exhibited handling
difficulties throughout the remainder of the flight. The captain took
over flying duties and the first officer began running emergency
checklists. The captain requested a diversion from the air traffic
controller; she first requested the nearest airport but quickly decided
on Philadelphia. The controller provided vectors to the airport with no
delay. The flight crew reported initial communications difficulties
because of the loud sounds, distraction, and wearing masks, but, as the
airplane descended, the communications improved. The captain initially
was planning on a long final approach to make sure they completed all
the checklists, but when they learned of the passenger injuries, she
decided to shorten the approach and expedite landing.
The NTSB materials group is working to estimate the number of cycles
associated with fatigue crack initiation and propagation in the No. 13
fan blade and to evaluate the effectiveness of inspection methods used
to detect these cracks.
Passengers praise Southwest Airlines pilot who
safely landed plane after one of its engines blew out. Pilot
Tammie Jo Shults was one of the first women to fly a fighter
jet for the U.S., and passengers on the stricken Southwest flight called
her a hero.

9 Killed in Military Plane Crash in Georgia

The Lockheed WC-130H Hercules transport plane (tail
number 65-0968) was destroyed when it impacted Augusta
Road (Highway 21) and burst into flames shortly after takeoff from
Savannah/Hilton Head International Airport, Georgia, USA.
All nine occupants suffered fatal
injuries. The aircraft was attached to the Puerto Rico Air National Guard, 156th
Airlift Wing (156 AW) and had been undergoing routine maintenance in
Savannah before heading to the Aerospace Maintenance and Regeneration
Group (AMARG) at Davis Monthan Air Force Base in Arizona.
The aircraft had been at Savannah for almost a month, since 9 April
2018, to undergo prescheduled fuel cell maintenance and unscheduled work
on engine number one by 156 AW maintenance personnel using the
facilities of the 165th Airlift Wing.
During takeoff roll, engine no.1 RPM (Revolutions Per Minute) fluctuated
and did not provide normal RPM when the captain advanced the throttle
lever into the flight range for takeoff. Approximately eight seconds
prior to aircraft rotation, engine no.1 RPM and torque significantly
decayed, which substantially lowered thrust. The fluctuation on roll and
significant performance decay went unrecognized by the crew until
rotation, when the captain commented on aircraft control challenges and
the aircraft veered left and nearly departed the runway into the grass
before it achieved flight. As the crew retracted the landing gear, they identified the engine no.1
RPM and torque malfunction and the captain called for engine shutdown.
However, the crew failed to complete the Takeoff Continued After Engine
Failure procedure, the Engine Shutdown procedure, and the After Takeoff
checklist as directed by the Flight Manual, and the aircraft's flaps
remained at 50 percent. Additionally, the captain banked left into the
inoperative engine, continued to climb, and varied left and right rudder
inputs. At an altitude of approximately 900 feet mean sea level and 131
knots indicated air speed, the captain input over nine degrees of left
rudder, the aircraft skidded left, the left wing stalled, and the
aircraft departed controlled flight and impacted the terrain on Georgia
State Highway 21. USAF AIB concluded, that the causes of the mishap were:

Mishap Pilot 1 - MP1’s improper application of left rudder, which
resulted in a subsequent skid below three-engine minimum
controllable airspeed, a left-wing stall, and the MA’s departure
from controlled flight.

Additionally, the Mishap Crew - MC’s failure to adequately prepare
for emergency actions, the MC’s failure to reject the takeoff, the
MC’s failure to properly execute appropriate after takeoff and
engine shutdown checklists and procedures, and the Mishap
Maintainers’ failure to properly diagnose and repair engine number
one substantially contributed
to the mishap.
The list of names released by the Puerto Rico National
Guard noted that all nine onboard had several years - and for some,
decades - of experience in the service.
1.Maj. Jose R. Roman Rosado, pilot
Rosado had 18 years of service in the National Guard. He is survived by
his wife and two sons. 2.Maj. Carlos Pérez Serra, navigator
Serra had 23 years of service. He is survived by his wife, two sons and
a daughter. 3.1st Lt. David Albandoz, co-pilot
Albondoz, a Puerto Rican native who had been living in Alabama before
the crash, had 16 years of service. He is survived by his wife and
daughter. 4.Senior Master Sgt. Jan Paravisini,
mechanic Paravisini had 21 years of service in the National Guard. He is survived
by two daughters and son. 5.Master Sgt. Jean Audriffred
Audriffred had 16 years of service prior to the crash and is survived by
his wife and two sons. 6.Master Sgt. Mario Brana, flight engineer
Brana had 17 years of service in the National Guard. He is survived by
his mother and daughter. 7.Master Sgt. Víctor Colon
Colón had 22 years of service before the crash. He is survived by his
wife and two daughters. 8.Master Sgt. Eric Circuns, loadmaster
Circuns had 31 years of service to his name and is survived by his wife,
two step-daughters and son. 9.Senior Airman Roberto Espada
Espada had three years of service and is survived by his grandmother.

LZ-AGR, експлоатиран от „Авиотех“ ООД, участва в провеждане на
демонстрационни полети на летателна площадка Казанлък. След неуспешно
кацане, при опит за преминаване на втори кръг, самолетът се срива и пада
в дере, непосредствено до източния край на пистата за излитане и кацане.
Самолетът е разрушен. Пилотът и двама пътника, намиращи се на борда са
невредими.
Комисията за разследване във връзка с безопасността сочи като
причина за реализиране на авиационното произшествие:
Несъобразяване с основни експлоатационни характеристики на самолета и
допуснати грешки в технологията на пилотиране от пилота на самолета.

Air-Accidents registered 3X-GHP, that took off from
Conakry at 10 hours 01 minutes to a mine field located near Lero,
Kankan, where the aircraft was expected to arrive at 11 hours 41 minutes
was carrying on board four (4) people including two (2) pilots: a German
and a Cape Verdian and two (2) technicians, including one Polish and one
Guinean. It made an enroute stop at Conakry on a flight from Cape Verde.
En route, the pilot encountered poor weather conditions with low clouds
and fog when the airplane struck the slope of a mountain and
disintegrated on impact.
All four occupants were killed.

Селскостопански самолет PZL
M-18 Dromader с рег. знак LZ-8012 извърши
принудително кацане и се преобърна в оризище на границата на Пловдивска
и Пазарджишка области. Пилотът Йосиф Карамфилов, на
67 г., има 40-годишен стаж, бил е сам и няма тежки
наранявания. Машината паднала на около 500 метра
от магистрала „Тракия” , в близост до 107-ия километър. Двигателят
отказал. Уменията на пилота са предотвратили разбиване на самолета на
пътното платно.
Самолетът се приземил в
полето, но заради меката почва машината се е преобърнала. В помощ на
пилота се притекъл случаен очевидец на инцидента. Не е имало
възпламеняване на машината. Самолетът се насочвал за кацане на летището
край Цалапица след привършване на пръскане на посеви. Машината е на 40
години. Води се разследване за изясняване на причините довели до
инцидента.
По информация на plovdiv24.bg пилотът
споделил, че двигателят отказал поради свършване на горивото.
LZ-8012 извършил
аварийно кацане и на летище Г. Оряховица на 27.01.2009 г. заради
изпаднало по време на полет ляво колело. Тогава нямало пострадали.

A Cessna 210L Centurion plane (PP-MMR) departed
Ourilandia do Norte airport, about 900 kilometers from the capital
Belem, around 1.30 am Friday and crashed in the community of Barra
Mansa, in Sao Felix do Xingu, southeast of Para. Five people were in the
aircraft. Three people died
in the accident -

(10 year-old), Evandro Geraldo Rocha Reis
(73) and Jose Goncalves de Oliveira
(72). Information indicates that the pilot attempted an emergency
landing after an engine failure. Illegal flight: According to the
Brazilian Aeronautical Registry, the aircraft had the certificate of
airworthiness expired since August of last year and, therefore, had no
authorization to fly. Initially the pilot Cristiano Rocha
and a passenger Robson Cintra survived the
crash of the plane, but later Cristiano
Felipe Rocha Reis
died
on 30.08.2018 at the Governor Otavio Lage Emergency Hospital in Siqueira
(Hugol), in Goiania. Several people lamented his death on social
networks, including singer Rodolffo, the duo with Israel, who was his
friend.
Tourist
plane crash kills four in Valais

On Friday afternoon, a small tourist Robin DR 400/180
aircraft (HB-KEE) belonged to the Sion Motor Flight
Group crashed near the summit of the Durand Glacier in the southwestern
Swiss canton of Valais, killing the pilot and three passengers on board.
The plane took off from Sion for a sightseeing flight then crashed under
unknown circumstances at 3350 meter altitude. The spot is about 150
meters below the Durand Pass. The aircraft was equipped with a beacon,
and after the crash, an alert was sent to Swiss air rescue service Rega
in Zurich. Rega notified the helicopter rescue service of Air Zermatt,
but rescuers could do nothing upon arriving at the scene. All
four occupants died in the
crash, the pilot (54 year-old), his son (21), Vaud nationals domiciled
in Valais, and a mother (59) of Denmark and her boy (20), residents of
Denmark. In August 2016, the plane had to land urgently due to an
alarming smoke release.
Small
plane crashed in Congo's Kasai province, 5 dead and 2 injured

The piston-engined Antonov 2R (9S-GFS), operated by
local airline Air Kasai, burst into flames after
crashing just after takeoff from Kamako for a 90-mile (150-kilometer)
flight to Tshikapa, located near the border with Angola. The plane was
on its way to Kabungu on the Angolan border. The An-2R developed engine
trouble shortly after takeoff. The pilot looked for a place to land, but
the problem got worse and worse. As the plane lost altitude, smoke
filled the cabin and the pilot emerge from the cockpit.
‘Follow the pilot!,’ said loudly the pilot, then opened an exit and
jumped out. A passenger succeeded also to jump out.
The pilot
and Pius Kabadi Tshiombe, a 53-year-old Congolese lay
evangelist who also works as a diamond dealer, were injured,
but survived the crash. The plane crashed into the brush three
kilometres (two miles) from Kamako. On board were 7 people,
5 passengers burned to death.
Among them are the conveyor Isaac,
a certain Top 13 boss, a lady Chantal
who came from Angola with her
children, a worker from the Baudouin fund transfer agency and a
trafficker from the square. The Tshiombe’s wife was unable to speak with
her husband for three days, but his first words by phone were filled
with praise to God. “I will never leave this God,” he
told her. “He is wonderful!”

an Aeromеxico Connect Embraer ERJ-190 (XA-GAL) impacted
airport terrain shortly after commencing takeoff from runway 03 at
Guadalupe Victoria International Airport (MMDO), Durango, and a
post-impact fire ensued. All 105 occupants survived but some of them
suffered injuries that required hospitalization. On Wednesday morning,
the airline tweeted that 64 people who were admitted to
local hospitals following the crash have since been discharged.
The aircraft arrived at Durango from Mexico City as flight 2430 at 14:11
hours local time, 23 minutes ahead of schedule. As the ERJ-190 was
prepared for the return flight there was a broken layer of cumulonimbus
clouds at 2500 feet. Airport weather information shows that a
thunderstorm had developed by 15:18 hours local time and the temperature
had dropped from 28°C to 20°C over the previous hour.
On board flight SLI2431 were two flight crew members, an observer, three
cabin crew members and 99 passengers. The aircraft commenced the takeoff
roll on runway 03 at 15:22 hours local time (planned departure time was
15:09 hours).
During the takeoff roll there were notable variations in airspeed and
wind direction. At 15:22:42 hours, the aircraft was accelerating through
airspeed of 146 knots. Wind at that time was 47 degrees (from the
right-hand side) at 33 knots. Eight seconds later the aircraft had
rotated and was climbing through 8 feet radio altitude at airspeed of
145 knots. By that time the wind had shifted to a crosswind: 103 degrees
at 11 knots. The aircraft reached a highest altitude of about 30 feet.
At 15:22:56 hours, the aircraft had descended to 19 feet and was caught
in a 22-knot tailwind of 30 degrees.
The aircraft then hit the ground to the left of the runway, left wing
first. Both engines broke away during the subsequent ground slide. The
aircraft came to rest about 380 meters past the runway threshold and 320
meters past the paved end of the runway, slightly to the left of the
extended centerline. The length of runway 03 is 2900 m (9514 ft).
On Sep 5th 2018 Mexico's DGAC reported in a press conference
first results into the investigation. There were three
flight crew on the flight deck, a first officer in training was pilot
flying in the right hand seat. The takeoff run began in a head wind
scenario, which quickly changed into a strong right crosswind and
ultimately a strong tail wind as result of a microburst. The
investigation so far has not found any technical (both engines were
operating normally until being separated from the wing) or human factors
issue contributing to the accident. With the flight data, confirmed by
meteorologists, simulator tests were conducted, no flight crew was able
to get through the scenario with a different outcome. However, the
training of the first officer under training had not been authorized and
was not carried out according to the required protocols. Although this
is not the cause of the accident, it will cause administrative response
by the DGAC to establish the responsibilities and apply according
sanctions.

Tobago plane (HB-EZW) crashed Saturday into a
forest in central Switzerland, killing a local family of four, 2 adults
and 2 children, who had just taken off from Kaegiswil (LSPG) to Basel
(LFSB). The aircraft went down Saturday morning near the town of
Hergiswil, which is on Lake Lucerne at the foot of Mount Pilatus.
Authorities had to send up a helicopter to extinguish the resulting fire
before rescue workers could get to the scene. There was no immediate
word on the possible cause of the crash, which was under investigation
by Swiss authorities.
This plane was a machine of the Pilatus
aircraft group, which had been used almost exclusively by employees of
the Pilatus aircraft factory. The deceased pilot is the chief pilot of
the Pilatus Aircraft Ltd. Reto Aeschlimann,
the other three victims are his wife and their two children (13 and
15-year-old). Pilatus CEO Oscar Schwenk stated his condolences and that
this accident will have an impact on the further testing in the PC-24
program, as the chief pilot was strongly involved in it.

край с. Цалапица. Няма пострадали при инцидента. Инцидентът е станал
около 18.25 часа м. вр. на 21 август 2018 г. край село Цалапица по време
на тренировъчен полет за медицинска евакуация с вертолет от състава на
24-та авиобаза в Крумово.
По време на полета се наложила рязка маневра,
за да се избегне ято птици, при която машината е закачила жици на
електропровод. Домакинства от град Септември са останали за часове без
ток и вода.
Тричленният екипаж е приземил вертолета на
летището край с. Цалапица. При огледа за щети се е наложило да бъдат свалени
витлата му, а вертолетът да бъде превозен с наземен транспорт до
летището в Крумоно, където да бъде отремонтиран.
Така от наличните 12 Кугъра във Военновъздушните сили на България
остават едва пет изправни. Нальотът на летците е повече от
жалък - по 4 часа годишно за някои от тях, споделя в интервю командирът
на авиобазата в Крумово. По случая се води разследване от Служба „Военна
полиция“.

44-8195008 извършил принудително кацане в овощна градина
северно от Шумен около 19 часа местно време. Сигналът за инцидента е
постъпил в ОДМВР и оперативния център на пожарната в 19:20. На борда
имало двама души, по неофициална информация баща (Валери
Димитров на 62 години) и син (Александър
Димитров на 27 години). Според някои източници и двамата са
правоспособни пилоти и дори инструктори. По време на инцидента
вятърът бил гръбен. Машината прелетяла две трети от пистата, но
не успяла да кацне. Пилотът решил да мине на втори кръг. В този момент
самолетът пропаднал вероятно заради скоростния срив. При приземяването
се ударил в дърво и повредил един от резервоарите, от който започнало
изтичане на гориво. Двамата мъже били настанени в шокова зала на МБАЛ
Шумен за оказване на медицинска помощ.
Дни след катастрофата 62-годишният пилот починал в резултат
на получените наранявания. Директорът на болницата в
Шумен споделя, че същият до последно е бил на апаратно дишане, с
множество наранявания и тежка травма на белия дроб.
Разследващите органи работят по версиите за човешка
грешка, техническа повреда или лоши метеорологични условия.
Най-вероятната от тях е появата на внезапен силен вятър.
Piper PA-44-180 е лек двумоторен самолет на американската компания Piper
Aircraft. Машината паднала край Шумен е модел 1980 година и закупена от
САЩ през 2018 година от първия ѝ собственик, германец. Самолетът се
управлява от един пилот и има места за трима пасажери.

Към 17,45 ч м. вр. в петък самолет
Cirrus SR22-G3, с регистрационни знаци LZ-SEV,
излита от летище Лесново. Лети на височина 3100 метра, след което
преминава през въздушното пространство на летище Варна и в 18,54 часа е
предаден за кацане на летище Балчик. Пилотът потвърждава, че е
осъществил контакт с летище Балчик и се насочва за кацане. Самолетът се
приземява твърдо, подскача, преобръща се през крило и уврежда. Не се
запалва. Според очевидци подходът бил с висока скорост, но бордовите
прибори са разбити и е трудно да се каже с каква скорост се е приземил.
Вятърът бил към 2-3 метра в секунда, страничен и леко пулсиращ, но
според разследващите, не той е причина за преобръщането.
Пилотът
Севдалин Вълнаров, единствен на борда,
загива при катастрофата.
Същият е роден през 1966 година, завършва Математическа гимназия в Русе,
след това ВНВТУ “Тодор Каблешков”, специалност “Транспортно
строителство” и има квалификация Строителен инженер. От 1989 год. до
1997 год. е офицер във войските на Министерство на транспорта. От 1998
год. до събитието е бил собственик и управител на Рубикон инженеринг
ООД, с предмет на дейност консултански услуги и строителен надзор.
Основната реализация на фирмата е в Железопътната инфраструктура. Бил е
председател на Българската асоциация на консултантските фирми в
железопътното строителство. Според някои медии има общ нальот от около
60 часа. Закупува самолета със съдружник и им е доставен от Испания на
02.08.2018. Cirrus SR22-G3 е с един двигател, 4-и
местен, подобрена версия, която съгласно извършените тестове дава
възможност за приземяване, дори при не особено прецизен подход. Ще
добавим само, че летище Балчик има особености, които внасят допълнително
напрежение при кацане дори при опитни летци. Разследването на
катастрофата продължава.

притежание на фирмата Air Mizia с базиране в Гривица
(LBGR), закачил и скъсал жици на далекопровод. Пилотът успял да овладее
машината и след три километра се приземил аварийно в слънчогледова нива
на метри от железопътен прелез до град Брусарци, Област Монтана.
Вертикалният стабилизатор получил увреждания. Няма пострадали, освен
неудобството че заради скъсаните жици няколко селища в региона останали
без ток. Полицията разследва случая. Образувано е досъдебно
производство. Води се разследване за изясняване причините довели до
инцидента.

On 30.08.2018 at around 10.30am local time an
Ethiopian Air Force de Havilland Canada DHC-6 Twin Otter 300
crashed near the town of Mojo and burst into flames. The aircraft was
reportedly en route from Dire Dawa-Aba Tenna D Yilma Airport (DIR/HADR)
to the Bishoftu-Harar Meda Air Base (HAHM), The aircraft, registered as
ET-AIU/808, MSN 822, dates back to 1985 when it first
entered service before in 2012 joining the Ethiopian Air Force fleet.
A crew of 3 and 15 passengers on
board all perished in the crash. No additional
information about weather conditions or a possible Mayday call prior to
the crash are available at this time due to the secretive nature of the
ongoing investigations.

A Boeing 737-800 (P2-PXE)
operated by Air Niugini came to rest in a lagoon during an attempted
landing at Chuuk International Airport in Micronesia. The aircraft
landed in the water about 145 metres off shore from the runway 04
threshold.
On board were 47 occupants. Eight people were hospitalized.
All the patients were males with head injuries, spinal cord injuries and
hip fractures. One passenger was
reported missing. His body was found by divers on October 1.
Flight PX73 flies from
Tokyo's Narita International Airport to Papua New Guinea's capital, Port
Moresby, via Pohnpei and Chuuk. The aircraft was approaching TKK in rain
with Cumulonimbus clouds moving at 800-1000 feet over the airport. Air
Niugini released a statement on the crash and said the weather was poor
with heavy rain and reduced visibility at the time of the incident.
After the aircraft came to rest, local boats and some United States Navy
divers went to the scene to rescue the occupants.
Pilot error led to an airliner ditching in a Pacific
lagoon last year, forcing passengers and crew to swim for their lives,
an official crash investigation released on 18 July 2019 found.
A Papua New Guinea Accident Investigation Commission (AIC) report into
the September 28 crash found the pilot and co-pilot ignored numerous
automated warnings while approaching the runway.
It said the pair missed "pull up" warning lights and continued the
landing attempt at Chuuk International Airport, even after bad weather
made them lose sight of the runway.
"Both pilots were fixated on cues associated with control inputs for the
landing approach, and subsequently were not situationally aware," AIC
chief commissioner Hubert Namani said.
A transcript revealed the dramatic final seconds in the cockpit before
the crash.
"(The) co-pilot called rapidly with high intonation: 'Too low! We're too
low! We're too low! We're too low!" it said.
The report said the man who died was not wearing a seatbelt and
suffered blunt trauma injuries to his head, which probably killed him
minutes after the crash.
It said Air Niugini had agreed to a number of changes, including
increased training for crews flying to Chuuk and tighter restrictions on
landing requirements at the airport.

An army helicopter crashed
in southwestern Afghanistan on Wednesday,
killing all 25 people on board, including a top
commander and the head of the provincial council key in fighting off a
Taliban attack in May, officials said. The copter was carrying two
pilots and 23 passengers.
Taliban insurgents fighting the Western-backed government said they shot
it down.
Two army helicopters were on their way from Anar Dara, Farah province
capital, to neighboring Herat when one lost control in low visibility
and crashed into a mountain. Among the passengers were
Nematullah Khalil, deputy
army corps commander for the western region, and
Farid Bakhtawar, the outspoken head of Farah's
provincial council. The other victims, apart from the crew, were
soldiers and council members.
Bakhtawar was a major figure in battling the Taliban in the battle for
the city of Farah which the militants besieged and threatened to take
over in May. The Taliban are seeking to remove the government and
reimpose strict Islamic law after their ouster by U.S.-led forces in
2001.

A private plane, Van's RV-10 (PR-ZMZ), crashed when it
was approaching to land in Patos de Minas Airport.
A family of 5 persons perished.
The military confirmed that they are a couple being
the doctor Marcos Nogueira Chagas, 45 years old and doctor Carla
Giannine Pereira Medina, 44 years old, besides three children, two girls
of 8 and 10 years and a boy of 12 years.
The aircraft took off from Brasilia and landed at Patos de Minas, from
where it would continue to Varginha, in the south of Minas. An
eyewitness informed that the aircraft was flying fast and low. "The
situation looked like panic. I saw the aircraft crossing the airfield, I
think he did everything to get back on track. I thought he would get
around to land. Then I heard the noise of the fall, unfortunately", he said.
The bodies were taken to the Legal Medical Institute of Patos de Minas.
Belongings of the victims were collected by the expert. Experts have
reported that Varginha's relatives are proceeding to Patos de Minas to
recognize the bodies.
Civilian Police teams were on site and assisted in the rescue work on
bodies. The Center of Investigation and Prevention of Aeronautical
Accidents (Cenipa) clarified that teams were sent to the place of the
crash.

central district of Istanbul. The
sergeant, who was wounded, later died
from the injuries sustained

The helicopter crashed in the middle of a residential area in the
Sancaktepe district of Istanbul, where a military base is situated on
the Asian side of the city.
The aircraft was carrying out training flights at the Samandira air base
in Istanbul.

2 US warplanes crash off Japan; 2
found, 5 missing

Japan/ 100 km (62.5 mls) SSE off Muroto Cape in Pacific Ocean
The two aircraft were carrying seven crew members in total, two in the
F/A-18 and five others in the KC-130,
when they collided and crashed into the sea south of the Muroto Cape on
Shikoku island in southwestern Japan. The cause of the crash is under
investigation. The airmen might have been taking part in a nighttime
midair refuelling exercise – a difficult manoeuvre used on long-range
flights – when the collision occurred.
Both F/A-18 crewmen were rescued by Japanese Maritime Self Defense
Forces. One later died.
The search for the 5 crew-members
of the KC-130, attached to the Marine Aerial Refuel Transport Squadron
152 (VMGR-152), was abandoned on December 11, 2018.

Four people were killed after the private Agusta Westland AW119
helicopter (RA-01908) crashed near Ulan-Ude, the capital of Siberia’s
Buryatia republic.
The department of the Russian Emergencies Ministry in the republic of
Buryatia confirmed the information. The helicopter belonged to a local
businessman.
Rescuers extinguished the fire of the helicopter, which broke out after
the accident.
